Question breakdown, scoring and interpretation Time to complete is about 5–10 minutes. Each question asks the respondent to rate a statement or behavior with "never/rarely", "sometimes", "often" , or "I don't know".

Question breakdown Each question belongs to one of three categories: motor skills, visual-spatial skills, and interpersonal skills.

Scoring and interpretation If the respondent selects "often" or "sometimes" for three or more of the motor skills questions, four or more of the visual-spatial skills questions, and three or more of the interpersonal skills questions, it is highly recommended that a more in-depth evaluation of the respondent.
